Agreement to Terms

By accessing or using the website at primemedicalhrt.com, scheduling appointments, or using any services provided by PRIME Medical Group PLLC ("PRIME," "we," "us," or "our"), you agree to be bound by these Terms and Conditions. If you do not agree to these terms, please do not use our website or services.

PRIME Medical Group PLLC is a licensed medical practice operating in the State of Illinois and the State of Florida, with locations in Chicago, IL and St. Petersburg (Tampa Bay), FL, and telehealth services available nationwide.


1. Medical Services Disclaimer

A. Not Emergency Care

PRIME Medical Group PLLC provides elective wellness, hormone optimization, and longevity medicine services. We are not an emergency medical provider. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.

B. Educational Information Only

The information provided on primemedicalhrt.com is for general educational purposes only and does not constitute medical advice, diagnosis, or treatment. All medical decisions should be made in consultation with a licensed healthcare provider.

C. No Physician-Patient Relationship

Visiting our website or submitting a contact form does not establish a physician-patient relationship. A physician-patient relationship is only established after a formal medical evaluation and acceptance as a patient by PRIME Medical Group PLLC.

D. Individual Results Vary

Results from hormone therapy, weight loss programs, peptide therapy, regenerative medicine, IV therapy, and all other services offered by PRIME Medical Group PLLC vary based on individual medical history, adherence to prescribed protocols, lifestyle factors, and genetic factors. No specific outcomes are guaranteed.

E. FDA Notice

Some treatments offered by PRIME Medical Group PLLC, including compounded medications and certain peptide therapies, may not be FDA-approved for specific indications. All treatments are prescribed by licensed medical providers based on clinical judgment following medical evaluation. Use of these treatments is voluntary and requires informed consent.


2. Eligibility & Patient Requirements

A. Age Requirement

Our services are available to individuals 18 years of age or older. We do not provide medical services to minors without explicit parental or guardian consent and in compliance with applicable state laws.

B. Geographic Availability

In-person services are available at our Chicago, IL and Tampa Bay, FL locations. Telehealth services are available in states where our providers are licensed to practice. It is your responsibility to confirm telehealth availability in your state prior to scheduling.

C. Medical Evaluation Required

All treatment programs require a medical evaluation and laboratory testing prior to initiation. PRIME Medical Group PLLC reserves the right to decline service to any individual who does not meet clinical criteria for treatment or where treatment would not be medically appropriate.

D. Accurate Information

You agree to provide accurate, complete, and current information during the intake process and throughout your care. Providing false or misleading medical information may result in termination of services and may have legal consequences.


3. Appointments & Cancellation Policy

A. Scheduling

Appointments are scheduled through our online booking system (OptiMantra) or by calling (888) 717-7463. Appointment availability is not guaranteed until confirmed by PRIME Medical Group PLLC.

B. Cancellation Policy

  • Cancellations must be made at least 24 hours prior to your scheduled appointment
  • Cancellations made less than 24 hours before the appointment may be subject to a cancellation fee
  • No-show appointments may be subject to a no-show fee
  • Repeated no-shows or late cancellations may result in termination of services

C. Late Arrivals

Patients arriving more than 15 minutes late for their appointment may be required to reschedule. Late arrivals do not extend appointment duration.

D. Telehealth Appointments

For telehealth appointments, you are responsible for ensuring a stable internet connection and appropriate technology. Technical difficulties on your end do not qualify for refunds or rescheduling without charge.


4. Payment Terms

A. Payment Methods

PRIME Medical Group PLLC accepts major credit cards, debit cards, HSA/FSA cards, and cash at our locations. Payment is due at the time of service unless other arrangements have been made in advance.

B. HSA/FSA Eligibility

Many of our services may be eligible for payment using Health Savings Accounts (HSA) or Flexible Spending Accounts (FSA). It is your responsibility to verify eligibility with your HSA/FSA administrator prior to payment.

C. Insurance

PRIME Medical Group PLLC is not currently in-network with any insurance providers. We are a fee-for-service practice. We provide documentation that may be submitted to your insurance provider for potential reimbursement, but we make no guarantees regarding insurance coverage or reimbursement.

D. Refund Policy

  • Consultation fees are non-refundable once a consultation has been completed
  • Unused portions of prepaid programs may be refunded on a pro-rated basis within 30 days of purchase
  • Medications and compounded prescriptions that have been dispensed are non-refundable
  • IV therapy and in-office treatment fees are non-refundable once treatment has been administered
  • Supplement and product purchases are subject to our separate return policy

E. Subscription & Membership Programs

If you enroll in a membership or subscription program, you authorize recurring charges to your payment method. Memberships may be cancelled with 30 days written notice. Cancellation does not entitle you to a refund of the current billing period.

F. Outstanding Balances

Outstanding balances are due within 30 days of the service date. Accounts with outstanding balances may be referred to a collections agency. You agree to pay all reasonable costs of collection, including attorney fees.


5. Compounded Medications & Prescriptions

A. Compounding Pharmacy

PRIME Medical Group PLLC works with licensed 503A and 503B compounding pharmacies to fulfill prescriptions. Compounded medications are prepared specifically for individual patients based on a valid prescription from a licensed provider.

B. FDA Status

Compounded medications are not FDA-approved drugs. They are prepared by licensed pharmacists under physician supervision in compliance with applicable federal and state laws. The safety, efficacy, and quality of compounded medications may differ from FDA-approved products.

C. Shipping & Delivery

Medications shipped to your home are sent via HIPAA-compliant, discreet packaging. Delivery times vary. PRIME Medical Group PLLC is not responsible for delays caused by shipping carriers, incorrect addresses provided by the patient, or customs issues.

D. Medication Storage

You are responsible for proper storage of prescribed medications as instructed by your provider. Improper storage that results in medication degradation does not qualify for replacement or refund.

E. Controlled Substances

PRIME Medical Group PLLC does not prescribe controlled substances via telehealth except in compliance with applicable federal and state law, including the Ryan Haight Online Pharmacy Consumer Protection Act.


6. Telehealth Terms

A. Consent to Telehealth

By scheduling a telehealth appointment, you consent to the use of electronic communications for medical consultation. A separate Telehealth Informed Consent form will be provided prior to your first telehealth appointment.

B. Technology Requirements

You are responsible for providing appropriate technology for telehealth consultations, including a device with camera and microphone capability and a stable internet connection.

C. State Licensing

Telehealth services are only available in states where PRIME Medical Group PLLC providers are licensed. It is your responsibility to confirm availability in your state.

D. Limitations of Telehealth

Telehealth consultations have inherent limitations compared to in-person visits. Certain examinations and procedures require in-person evaluation. Your provider will advise you if an in-person visit is necessary.


7. Fitness for Duty

A. First Responders & Military

PRIME Medical Group PLLC provides services to active duty military, veterans, law enforcement, firefighters, EMS professionals, and other first responders. Treatment received at PRIME Medical Group PLLC is provided under physician supervision and in accordance with applicable standards of care.

B. Patient Responsibility

It is the sole responsibility of the patient to disclose any treatment received at PRIME Medical Group PLLC to relevant occupational health providers, employers, or regulatory bodies as required by their specific employment, branch of service, or agency requirements. PRIME Medical Group PLLC does not provide fitness for duty certifications.


8. Intellectual Property

A. Website Content

All content on primemedicalhrt.com, including text, images, logos, graphics, videos, and other materials, is the property of PRIME Medical Group PLLC and is protected by applicable copyright, trademark, and intellectual property laws.

B. Limited License

We grant you a limited, non-exclusive, non-transferable license to access and use our website for personal, non-commercial purposes. You may not reproduce, distribute, modify, or create derivative works from our content without express written permission.

C. Trademarks

"PRIME Medical Group," the PRIME logo, and all related marks are trademarks of PRIME Medical Group PLLC. Unauthorized use of our trademarks is prohibited.


9. User Conduct

By using our website, you agree not to:

  • Submit false or misleading information

10. Reviews & Testimonials

A. Authenticity

All patient testimonials and before/after results featured on our website reflect the experiences of real patients. Individual results vary and are not guaranteed.

B. Reviews

If you submit a review or testimonial, you grant PRIME Medical Group PLLC a non-exclusive, royalty-free license to use, reproduce, and display your review in our marketing materials. Reviews may be edited for length and clarity but will not be altered in meaning.

C. HIPAA & Reviews

We will never disclose protected health information in connection with patient reviews or testimonials without explicit written authorization from the patient.


11. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, PRIME MEDICAL GROUP PLLC, ITS OFFICERS, DIRECTORS, EMPLOYEES, AND AGENTS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM:

  • Your use of or inability to use our website or services
  • Any errors or omissions in our website content
  • Unauthorized access to your personal information
  • Any interruption or cessation of our services
  • Results or outcomes of medical treatment

OUR TOTAL LIABILITY TO YOU FOR ANY CLAIMS ARISING FROM THESE TERMS OR YOUR USE OF OUR SERVICES SHALL NOT EXCEED THE AMOUNT YOU PAID TO US IN THE THREE MONTHS PRECEDING THE CLAIM.


12. Indemnification

You agree to indemnify, defend, and hold harmless PRIME Medical Group PLLC and its officers, directors, employees, agents, and affiliates from any claims, damages, losses, liabilities, costs, and expenses (including reasonable attorney fees) arising from:

  • Your use of our website or services
  • Your violation of these Terms and Conditions
  • Your violation of any applicable law or regulation
  • Any false or misleading information you provided

13. Governing Law & Dispute Resolution

A. Governing Law

These Terms and Conditions are governed by the laws of the State of Illinois, without regard to its conflict of law provisions. For patients receiving services in Florida, applicable Florida law will also apply.

B. Dispute Resolution

Any disputes arising from these Terms and Conditions or your use of our services shall first be submitted to good-faith mediation. If mediation is unsuccessful, disputes shall be resolved through binding arbitration in Cook County, Illinois, in accordance with the rules of the American Arbitration Association.

C. Class Action Waiver

You waive any right to participate in a class action lawsuit or class-wide arbitration against PRIME Medical Group PLLC.

D. Time Limitation

Any claim arising from these Terms and Conditions must be brought within one (1) year of the event giving rise to the claim, or it is permanently barred.


14. Modifications to Terms

PRIME Medical Group PLLC reserves the right to modify these Terms and Conditions at any time. Material changes will be communicated by:

  • Updating the "Last Updated" date at the top of this document
  • Posting a notice on our website
  • Email notification for significant changes

Your continued use of our website or services after changes are posted constitutes acceptance of the updated terms.


15. Severability
